New hope for heart patients in Egypt with innovative surgical approach

Revolutionary technique simplifies up to four heart surgeries into one

It is estimated that 40% of patients in Egypt who suffer from a cardiovascular disease (CVD) will face premature mortality[i]. This worrying statistic means that it is imperative that CVD patients have access to viable ways of treating such cardiac issues. Obesity, a lack of physical activity and having raised blood pressure are just some of the key attributable risk factors that can trigger the onset of non-communicable diseases (NCD) such as CVD.

The rising number of CVDs in the region has reached upsetting levels for families, individuals and governments alike. As heart problems begin developing, the risk of new complications increase which can result in these problems growing to affect multiple elements of the heart’s functioning, requiring multiple and often specialist surgeries to resolve the issue.

Revolutionary technique avoids numerous surgeries for heart patients

Dr Toufan Bahrami, a consultant cardiac surgeon at Royal Brompton & Harefield Hospital Specialist Care, has developed a pioneering technique to minimise the time heart patients with complex cases spend undergoing surgery and recover faster, which could help to combat the growing mortality rate caused by CVDs in Egypt.

Dr Bahrami developed a revolutionary heart procedure that is suited to heart patients needing multiple surgical procedures. Patients suffering from multiple life-threatening heart problems can have up to four surgical procedures completed during a single operation, using minimal access techniques.

Speaking with Dr Bahrami about this revolutionary technique, he explains “At Harefield Hospital we have developed a procedure that traditionally would be up to four different heart surgeries - fixing the mitral valve, tricuspid valve, ablation, and left atrial appendage closure. Now this all can be done in one surgery with quicker recovery time."

The procedure is very beneficial for the thousands of patients each year who develop multiple heart problems in Egypt – such as serious cases of high blood pressure, stroke, or irregular heart rhythms - it spares patients from undergoing multiple separate operations. It also helps reduce the disruption to their daily life, allowing them to get back to their families, hobbies and work much more quickly, rather than undergoing another heart operation within a short space of time.

In this procedure, Dr Bahrami accesses the heart via a handful of small cuts instead of the standard large incision through the breastbone. He highlighted “Depending on the condition of the patient, the surgery is either conducted ‘off-pump’ while the heart is still beating, or a heart-bypass machine is used to stop the heart for as little as 10 minutes compared to 30 minutes in traditional surgery.”

The instruments used in video-assisted surgical procedures are also different to those used in traditional surgery and it takes many years of training to develop the skills needed. The procedure is both very delicate and sophisticated with "the operation being video-assisted, utilising a state-of-the-art 3D camera and a large high-definition monitor, which gives an extremely clear view inside the body, instead of using direct vision, which requires a larger access port. The aortic valve (the valve in the heart that holds the oxygen-rich blood which is pumped out to the body) is accessed endoscopically with three small access holes. One for the camera, one to put a clamp on the aorta, and one to expose the valve.”

Mr Bahrami adds “Every week, we do at least two of these types of surgery. The number of times we have performed this procedure has made us outstanding. We have done mini-access heart surgery here since 2001. The programme that we are doing today has at least fifteen years’ experience behind it"

The surgery takes about three hours and the patient typically stays at the hospital to recover for around five to seven days.

Comparing the mini-access procedure to the more standard sternotomy approach, Dr Bahrami clarifies “The length of stay in the hospital is mainly the same; what makes a difference is afterwards. The integrity of the chest is preserved, so therefore the recovery is much quicker. Those who have a sternotomy can suffer for around two to three months. They often cannot go back to work nor drive for eight weeks at least. They can be very slow in recovery.”

He explains that particularly for obese patients, with traditional surgery they could spend “months” in the hospital. But with minimally invasive surgery they can go home after a much shorter period as their recovery is so much better – in as little as three days. Patients’ symptoms and quality of life is improved by fixing up to four issues in one go: the mitral valve, tricuspid valve, ablation, and left atrial appendage closure.

Dr Bahrami comments; “The whole operating theatre team – anaesthetists, nursing staff, and perfusionists – have been trained and are experts in this revolutionary technique. It’s not just one person who knows how to do the procedure or how it works – there is a whole team who are skilled to do it. They have the skills and are used to it.”

وتختلف الأدوات المستخدمة في العمليات الجراحية بمساعدة الفيديو عن الطرق المُتبعة في الجراحات التقليدية، كما أنها تستغرق سنواتٍ طويلة من التدريب لتطوير المهارات اللازمة. وتمتاز التقنية الجديدة بتطوّرها ودقتها الفائقة؛ حيث تعتمد على مساعدة الفيديو، باستخدام كاميرا متطورة ثلاثية الأبعاد وشاشة عرض كبيرة عالية الدقة، مما يوفر رؤية واضحة للغاية داخل الجسم، بدلاً من الاعتماد على الرؤية المباشرة التي تتطلب منفذ وصولٍ أكبر. ويمكن الوصول إلى الصمام الأبهري، الذي يحمل الدم الغني بالأكسجين لضخه في الجسم، باستخدام منظارٍ مزوّد بثلاثة ثقوب صغيرة، يكون أولها مخصصاً للكاميرا، والآخر لوضع المشبك على الشريان الأبهر، وآخر لفتح الصمام".

وقال الدكتور بهرامي: "نقوم كل أسبوع بإجراء عمليتين من هذه الجراحات على الأقل، وقد اكتسبنا خبرة واسعة ومهارات عالية بعد استخدام التقنية الجديدة مرات متعددة في العمليات الجراحية، ولم أسمع مسبقاً بأن مريضاً تمكن من إصلاح الصمام التاجي والصمام ثلاثي الشرفات وإجراء استئصال موضعي من خلال عملية جراحية واحدة، حيث يخضع المريض في العادة لعلاج الصمام التاجي أولاً، وتبقى الأجزاء الأخرى دون معالجة". 

التعافي بوتيرة أسرع

يستغرق إجراء العملية باستخدام التقنية الجديدة حوالي ثلاث ساعات، ويبقى المريض في المستشفى بغرض التعافي لمدة تتراوح بين 5-7 أيام في العادة.

وتعليقاً على المُقارنة بين العمليات الجراحية التي تتطلّب الحد الأدنى من التدخل الجراحي، ونهج المبضع الجراحي التقليدي، قال الدكتور بهرامي: "إن مدة الإقامة في المستشفى بشكل عام هي نفسها، ولكن يمكن اكتشاف الفرق الإيجابي والملموس بوضوح لاحقاً. وتضمن التقنية الجديدة الحفاظ على سلامة الصدر، وبالتالي يكون الشفاء أسرع بكثير مقارنة بالجراحة التقليدية، حيث أن العمليات باستخدام المبضع الجراحي التقليدي تتطلّب توفير الرعاية والعناية للمرضى لمدة تتراوح بين 2-3 أشهر، وغالباً لا يمكنهم العودة للعمل أو قيادة السيارة قبل 8 أسابيع على الأقل من الخضوع لها، ناهيك عن أن وتيرة التعافي تكون بطيئة للغاية".

ويؤكد الدكتور بهرامي أن المرضى الذين يعانون من السمنة المفرطة قد يُضطرون لقضاء عدة أشهر في المستشفى بعد إجراء العملية الجراحية التقليدية، ولكن التقنية الجديدة تضمن لهم تعافياً سريعاً لأنها تتطلب الحد الأدنى من التدخل الجراحي؛ ويمكن للمريض بعدها العودة لمنزله بعد فترة أقصر، والتعافي بسرعة أكبر في غضون ثلاثة أيام. كما تسهم التقنية الجديدة في تحسين الأعراض المُصاحبة للمرضى ونوعية حياتهم عن طريق إصلاح 4 مشكلاتٍ قلبية دفعةً واحدة، تشمل إصلاح الصمام التاجي، والصمام ثلاثي الشرفات، والاستئصال الموضعي، وإغلاق مُلحق الأذين الأيسر.

$1 million grant in digital skills - Google.org is regranting Injaz, an NGO that specializes in youth training, $1 million to scale digital skills trainings in Arabic, to reach a further 100,000 people in 2020.

The Google for MENA event was attended by hundreds of people representing government, business leaders, developers and entrepreneurs.

In 2018, Google launched an Arabic digital skills program, “Maharat min Google,” which has reached more than 500,000 young people, women and underprivileged students in the MENA region. 130,000 of those were able to find jobs, grow their careers and their businesses.

A 2019 research by Public First research company estimates that 89% of people in the UAE and Saudi Arabia use Search once a month to learn a new skill, and 80% of online Egyptians use Google Search because it provides new information not available by other means.

يستقطب المجمع الفاخر مونت ري غولف آند كانتري كلوب الذي يقع على مساحة تقدر بألف فدان في منطقة "الغارف الشرقية" الريفية المذهلة في البرتغال، المستثمرين من المملكة العربية السعودية عبر الفرص الجذابة التي يقدمها لهم. 

وفي هذا الإطار، تم تنظيم حملة ترويجية في كل من الرياض وجدة كجزء من خطة مونت ري التسويقية في الشرق الأوسط التي أقيمت ضمن سبعة تواريخ مختلفة، لعرض أبرز المزايا التي يتمتع بها هذا المجمع الذي يضم مجموعة من الفلل الراقية التي تبلغ تكلفتها ملايين الدولارات، فضلاً عن المرافق الحصرية ووسائل الترفيه والراحة بما فيها ملعب "جاك نيكلاوس آند كانتري كلوب للغولف" الحائز على العديد من الجوائز المرموقة. 

منذ ستينيات القرن الماضي، كانت منطقة "الغارف" من أبزر المناطق الجاذبة للمستثمرين والمصطافين من منطقة الخليج عموماً والمملكة العربية السعودية على وجه التحديد، نظراً للحوافز الضريبية والتسهيلات المرتبطة بالحصول على التأشيرة، لذا فقد اكتسبت شهرة واسعة كوجهة مثالية لشراء منزل ثان أو منزل قضاء العطلات دون أي مؤشر على وجود تباطؤ أو ركود اقتصادي. 

وبهذه المناسبة، صرّح ديكلان ماكنوتن، المدير التجاري لدى "مونت ري" قائلاً: "تعد البرتغال وجهة جاذبة للمستثمرين السعوديين بسبب مرونة وديناميكية السوق العقاري فيها، ووجود عائدات مضمونة على الاستثمار، وانخفاض سعر المتر المربع فيها مقارنةً مع غيرها من الدول الأوروبية، فضلاً عن المناخ الاستثماري العام الآمن والمستقر، بالإضافة إلى عدم وجود ضريبة على الميراث وعلى الإقامة غير الدائمة، مع منح "التأشيرة الذهبية" التي تخول صاحبها الحصول على إقامة طويلة الأمد في البرتغال".

يعد برنامج "التأشيرة الذهبية" الذي أطلقته الحكومة البرتغالية قبل عدة سنوات الطريق الأسرع للمستثمرين الأجانب من خارج دول الاتحاد الأوروبي للحصول على تصريح إقامة طويلة الأمد في البرتغال شريطة الاستثمار في صناديق عقارية أو استثمارية مطابقة للشروط بقيمة تتجاوز 350,000 يورو (نحو 414,000 دولار أمريكي) مع وجود شرط واحد يتمثل في الإقامة في البرتغال لمدة سبعة أيام في العام على الأقل.

وأضاف ماكنوتن: "بمجرد الحصول على تصريح الإقامة الدائم، سيتمكن المستثمرون وأفراد أسرهم المباشرين من الدخول والعيش في البرتغال وكذلك السفر والتنقل بحرية بين 26 دولة أوروبية ضمن منطقة الشنغن. وبعد مضي ست سنوات، يصبحون مؤهلين للحصول على الجنسية البرتغالية. 

وتعد الإصلاحات الضريبية من الحوافز الرئيسية الأخرى التي تقدمها الحكومة البرتغالية لجذب المستثمرين الأجانب، بما فيها خفض بعض الضرائب والإعفاء من دفع ضرائب أخرى مثل ضريبة الميراث والتصرف في الأصول ودخل الأفراد من الرواتب والمعاشات التقاعدية وذلك للمقيمين بشكلٍ غير دائم. 

وأكمل ماكنوتن بالقول: "يمكن للمستثمرين من المملكة العربية السعودية المقيمين بشكلٍ غير دائم في البرتغال الحصول على إعفاء من دفع ضريبة الدخل لمدة قد تصل إلى عشر سنوات بغض النظر عن البلدان التي تشكل مصدراً لدخلهم سواءً داخل البرتغال أو من دول مجلس التعاون الخليجي أو أي دولة أخرى في العالم". 

هذا ويضم منتجع مونت ري فلل ميرادورو فيليج التي تقع في قلب المجمع، وتضم 37 فيلا فاخرة مؤلفة من ثلاث غرف نوم توفر كل واحدةٍ منها إطلالات خلابة على المحيط الأطلسي والبحيرات والممرات المائية التي يضمها المنتجع. تبدأ أسعار الفلل المفروشة بالكامل من 1.32 مليون دولار أمريكي (ما يعادل 1.2 مليون يورو)، في حين يصل سعر المساكن المؤلفة من غرفتين وثلاث غرف نوم والتي تتميز بوجود مدخل خاص لها وبركة سباحة وصالة للياقة البدنية إلى 912,000 دولار أمريكي (نحو 825,000 يورو). 

يوجد ضمن مجمع "مونت ري غولف آند كانتري كلوب" الذي يقع بين جبال "سيرا دو كالديرو" والمحيط الأطلسي بركة سباحة، صالة للياقة البدنية مجهزة بالكامل، سبا، ملاعب تنس، منطقة ألعاب للأطفال، بالإضافة إلى عدد من المطاعم الراقية والحانات، وخدمة الكونسيرج المتوفرة على مدار الساعة وخلال أيام الأسبوع، ويتميز "مونت ري" عن غيره بوجود ملعب جاك نيكلاوس للغولف، وهو ملعب الغولف الوحيد في البرتغال الذي يضم 18 حفرة. 

واختتم ماكنوتن قائلاً: "يقدم مجمع مونت ري فرصاً ثمينة للسعوديين الباحثين عن شراء منزل للعطلات في أوروبا أو للمستثمرين الذين يتطلعون لشراء عقار فاخر كمشروع استثماري من خلال عرضه للإيجار". 

The UAE says ‘oui’ to European cream as imports from France increase to over 3,500 tonnes

The number of tonnes of French cream imported to the UAE increased by a CAGR of 15.3% between 2010 and 2018, with the value of imports increasing to over AED36 million.

Dubai, United Arab Emirates, 27th November 2019: The UAE’s imports of European cream increased to 8,702 tonnes in 2018 with France exporting 3,533 tonnes alone, according to the latest research from CNIEL, the French Dairy Board.

The value of French cream exported to the UAE in 2018 was valued at over AED36 million, an increase of 18% on 2017 and a compound annual growth rate (CAGR) of 15.3% since 2010.

Marie-Laure Martin, International Project Manager from CNIEL “The UAE and France enjoy a strong bond thanks to business and cultural links. French food exports, particularly cream, has helped further endorse the relationship between both countries as it grows in popularity from both a commercial perspective, through the increasing number of hotels ahead of Expo 2020, and from residents who are using the finest produce when baking or cooking meals at home.”

In addition to the UAE, several other countries in the Middle East have seen the popularity of European dairy exports increase in the last decade. Saudi Arabia imported 12,921 tonnes of EU cream, totalling over AED147 million. While Kuwait and Lebanon have seen this value of EU cream imports in 2018 increase to AED28.8 million and AED43.8 respectively.

However, it was not just EU finest cream tantalising the taste buds of UAE residents and visitors. The value of European cheese imported by the UAE increased during the same period by 8.5% to AED 383 million with 18,311 tonnes imported, among which 3,081 tonnes of French cheese.

Saudi Arabia imported AED534 million of European cheese last year, a 8.2% CAGR since 2010 while Kuwait and Lebanon imported AED373.8 million and AED108.8 respectively.

“In terms of French cheese, many people know Camembert, Roquefort and Brie however we want everyone to become familiar with the likes of Morbier, Fourme d’ambert as well as Tomme de Savoie, and Comté,” said Martin.

European butter imports in 2018 by the UAE totaled 4,294 tonnes with 1,147 tonnes for the French only, with a total value of over AED95.7 million, denoting a CAGR of 13.1% since 2010.




European butter exports have also proven popular in Saudi Arabia, Lebanon and Kuwait, with the countries witnessing a CAGR of 21.1%, 5.8% and 10.3% respectively from 2010.




“The evolving tastes of Middle East and Levant residents and tourists has aided the growth of French dairy exports, which in turn is piquing the interest of more French exporters who are keen to grow the market by offering an even greater variety of produce,” added Martin.




The French Dairy Board were recently in Dubai where they invited six of the leading women in the pastry world from the UAE, Saudi Arabia and France from some of Dubai’s leading hotels and bakeries to discuss the merits of French cream.




During the roundtable a range of topics were discussed ranging from sustainability, sourcing materials, teamwork and creating the perfect taste using the finest ingredients.

دبي، الإمارات العربية المتحدة، 27 نوفمبر 2019: وصل حجم واردات دولة الإمارات العربية المتحدة من الكريمة الأوروبية إلى 8,702 طن عام 2018، منها 3,533 طن من فرنسا وحدها، وذلك وفقاً لمجلس منتجات الألبان الفرنسي (CNEIL).

كما تجاوزت قيمة الكريمة الفرنسية المصدرة إلى دولة الإمارات العربية المتحدة عام 2018 حاجز الـ 36 مليون درهم إماراتي، بزيادة قدرها 18% عن عام 2017 بمعدل نمو سنوي مركب نسبته 15.3% منذ عام 2010. 

وتعليقاً على ذلك، قالت ماري لاوري مارتن، مديرة المشروع الدولي في مجلس منتجات الألبان الفرنسي: "تتسم العلاقات الثنائية بين الإمارات وفرنسا بأنها راسخة ومتينة بفضل الروابط التجارية والثقافية، كما تعززت هذه العلاقات من منظور الأعمال والتجارة من خلال نمو الصادرات الفرنسية من الأغذية لاسيما منتجات الكريمة التي تشهد زيادة في الطلب عليها من قبل الفنادق التي تستعد لمعرض إكسبو 2020 دبي، كما تحظى منتجات الأغذية الفرنسية ذات القيمة العالية بإقبال واسع من قبل سكان دولة الإمارات الذين يستخدمونها في صناعة المخبوزات وتحضير الطعام في المنزل". 

وبالإضافة إلى دولة الإمارات، شهدت الصادرات الأوروبية من منتجات الألبان زيادة ملحوظة إلى عدد من دول الشرق الأوسط الأخرى، حيث استوردت المملكة العربية السعودية 12,921 طناً من الكريمة الأوروبية تجاوزت قيمتها 147 مليون درهم إماراتي، بينما شهدت الكويت ولبنان ارتفاع قيمة وارداتها من الكريمة الأوروبية عام 2018 إلى 28.8 مليون درهم و43.8 مليون درهم على التوالي.

وفضلاً عن نمو حجم صادرات الألبان الأوروبية إلى المنطقة، شهدت منتجات الأجبان هي الأخرى زيادة في قيمة صادراتها إلى دولة الإمارات خلال الفترة ذاتها بنسبة 8.5% لتسجل 383 مليون درهم، حيث وصل حجمها إلى 18,311 طن، منها 3,081 طن من الأجبان الفرنسية. 

وعلى مستوى المملكة العربية السعودية، بلغت قيمة واردات الأجبان الأوروبية العام الماضي 534 مليون درهم بمعدل سنوي مركب نسبته 8.2% منذ عام 2010، بينما وصلت قيمة واردات الكويت ولبنان من هذه الأجبان إلى 373.8 مليون درهم و108.8 مليون درهم على التوالي. 

وأضافت مارتن قائلة: "على مستوى الأجبان الفرنسية، يعرف الكثير من الناس أنواع مختلفة من الأجبان على غرار كاميمبرت، روكيفور، وجبنة بري، ولكننا نرغب في الترويج لأنواع أخرى من الأجبان ذات المذاق الرائع مثل "موربييه"، "فورمي دي أمبرت"، وكذلك "تومي دي سافوي" و"كومتي".

وفي سياقٍ متصل، بلغ حجم صادرات الزبدة الأوروبية إلى دولة الإمارات 4,294 طن منها 1,147 طن من الزبدة الفرنسية وحدها بقيمة تزيد عن 95.7 مليون درهم إماراتي وبمعدل نمو سنوي مركب نسبته 13.1% منذ عام 2010. كما عززت الصادرات الأوروبية من الزبدة نموها في المملكة العربية السعودية ولبنان والكويت، بمعدل نمو سنوي مركب قدره 21.1% و5.8% و10.3% على التوالي منذ عام 2010.

واختتمت مارتن: "إن هذا النمو الذي حققته منتجات الألبان والأجبان والزبدة الفرنسية جاء بفضل الذوق الرفيع لسكان الشرق الأوسط والسياح على حدٍّ سواء ورغبتهم في تذوقها واختيارها، وهو ما أدى إلى تشجيع المصدّرين في فرنسا على زيادة صادراتهم وتنويع منتجاتهم وتقديم الأفضل".

تجدر الإشارة إلى أن مجلس منتجات الألبان الفرنسي أقام مؤخراً في دبي حدثاً مميزاً استضاف فيه ست من أمهر الطاهيات في صناعة المعجنات والحلويات من الإمارات والسعودية وفرنسا، تم التطرق فيه إلى أبرز ميزات وخصائص منتجات الكريمة والأجبان الفرنسية، بالإضافة إلى بعض المواضيع الأخرى مثل الاستدامة، توفير المصادر، العمل الجماعي، وتقديم أفضل الأصناف والأطباق من الحلويات والمعجنات باستخدام أجود المكونات والمواد من فرنسا.
