Lack of regional research into precision medicine may hinder targeted therapies for cancers in the UAE

Experts highlight that Middle Eastern populations are underrepresented in disease research


UAE Vision 2021 aims to reduce cancer-related deaths, which WHO estimated to be 25.6 deaths per 100,000 population in the UAE in 2017


Barriers to providing precision medicine in the Arab world to be discussed at MEDLAB Congress taking place from 4 - 7 February 2019 in Dubai




Dubai, United Arab Emirates, 12th December 2018: Ahead of the upcoming MEDLAB Exhibition & Congress, the MENA region’s largest medical laboratory event, experts are highlighting the need for advancing genetic and disease research in the Arab world in order to provide targeted gene therapies for certain cancers for citizens in the UAE.




This is in line with the UAE Vision 2021 National Agenda, which aims to reduce cancer-related deaths in the country, with the World Health Organisation (WHO) estimating 25.6 deaths per 100,000 population in the UAE in 2017. To achieve this aim, the UAE government regularly launches national awareness and preventative campaigns within the framework of ‘Itmenan’ which involves the ‘Universal Periodic Examination’ and ‘Early Detection of Cancer’ initiatives adopted by the Council of Ministers.




Targeted therapies, or precision medicine, is defined as tailoring of medical treatment to the individual characteristics of each patient. It is an emerging approach for disease treatment and prevention that takes into account individual variability in genes, environment, and lifestyle for each person. For example, for cancer treatments, based on genetic testing, a physician can choose the most effective chemotherapy based on the mutation or biomarker identified.




According to the WHO, while 70% of deaths from cancer occur in low and middle-income countries, only one in five low and middle-income countries have the necessary data to drive cancer policy. More than 90% of high-income countries reported available treatment services compared to less than 30% in low-income countries. While a portion of countries in MENA do have a high GDP, they are still considered developing economies according to the United Nations.




Commenting, Dr Sara Sorrell, Consultant Family Medicine, Intercare Health Center, Abu Dhabi, UAE, said: “It is well recognised that Middle Eastern populations are underrepresented in disease research and, to date, there is no research to indicate the impact of precision medicine in the UAE. But I think that this is still premature as we don’t even have enough data on the general population genetics of this part of the world, so this is really the first step.




“For instance, if a different mutation is found to cause a particular cancer here, compared to European descent populations, this may present a novel target for new cancer therapies for local populations. Or, if a new drug comes on the market, what’s the effect in the local population? Therefore, advancing genetic and disease research in the UAE and the Arab world is very important. For this to occur, there needs to be both a focus on funding research as well as formulating policies around research and healthcare innovation to protect patients, scientists, and physicians,” she added.




Molecular diagnostics and genetics experts from the MENA region will gather at the upcoming MEDLAB Congress taking place from 4 - 7 February 2019 at Dubai World Trade Centre to discuss topics such as barriers to providing precision medicine in the Arab World, strategies for the prevention of genetic diseases, and Next-Generation Sequencing in oncology, among others.




Commenting on the importance of scientific exchange in the regional medical laboratory industry, Rejoy Penacerrada, Conference Director, MEDLAB Congress, said: “The conferences provide an important platform for the region’s medical laboratory community to engage in discussion that will promote the role of genetic and population research in advancing the health of citizens across the region. This is led by the intense interest in precision medicine, with the region’s laboratories now actively expanding their test menus for more personalised diagnostic services.”

العوائق التي تحول دون توافر الطب الدقيق في الوطن العربي ستتم مناقشتها في مؤتمر ميدلاب الذي تنعقد جلساته في الفترة من 4 إلى 7 فبراير 2019 بدبي

دبي، الإمارات العربية المتحدة، 12 ديسمبر 2018: قبل الدورة المقبلة من معرض ومؤتمر ميدلاب، الذي يعتبر أضخم حدث للمختبرات الطبية في منطقة الشرق الأوسط وشمال إفريقيا، يُبرز الخبراء الحاجة إلى تطوير أبحاث الجينات الوراثية والأمراض في الوطن العربي من أجل توفير علاجات جينية مستهدفة لأنواع معينة من السرطان في دولة الإمارات العربية المتحدة. 

ويتماشى هذا مع الأجندة الوطنية لرؤية الإمارات 2021 التي تهدف إلى خفض معدلات الوفيات المرتبطة بالسرطان في البلاد، في ظل تقديرات من منظمة الصحة العالمية تشير إلى حدوث 25.6 حالة وفاة بين كل 100 ألف نسمة بدولة الإمارات في عام 2017. ولتحقيق هذا الهدف، تطلق حكومة الإمارات بصفة منتظمة حملات وطنية للتوعية بالأمراض والوقاية منها في إطار برنامج "اطمئنان" الذي يشمل مبادرتي "الفحص الدوري الشامل" و"الكشف المبكر عن السرطان" اللتين اعتمدهما مجلس الوزراء.

وتُعرف العلاجات المستهدفة أو الطب الدقيق بأنها تفصيل المعالجة الطبية وفق السمات الفردية لكل مريض، كما أنها تعتبر منهجًا ناشئًا لعلاج الأمراض والوقاية منها يأخذ بعين الاعتبار التباين الفردي في الجينات والبيئة ونمط الحياة لكل شخص. وعلى سبيل المثال، بالنسبة لعلاجات السرطان، يمكن للطبيب الاستناد إلى الاختبارات الجينية للتوصل إلى العلاج الكيميائي الأكثر فاعلية بناءً على الطفرة الجينية أو المؤشرات الحيوية التي تم اكتشافها.

ووفقًا لما ذكرته منظمة الصحة العالمية، في حين أن 70% من الوفيات الناجمة عن السرطان تحدث في الدول المنخفضة والمتوسطة الدخل، فإن دولة واحدة من بين كل خمس دول منخفضة ومتوسطة الدخل تمتلك البيانات اللازمة لتعزيز سياسة علاج أمراض السرطان. وأفادت أكثر من 90% من الدول المرتفعة الدخل عن توافر خدمات العلاج مقارنةً بأقل من 30% في الدول المنخفضة الدخل. وفي حين أن جزءًا من الدول الموجودة في منطقة الشرق الأوسط وشمال إفريقيا تسجّل ناتجًا محليًا إجماليًا مرتفعًا، فإنها لا تزال تعتبر اقتصادات نامية وفقًا لمنظمة الأمم المتحدة. 

وتعليقًا على ذلك، قالت الدكتورة سارة سوريل، استشارية طب الأسرة بمركز إنتركير الصحي في العاصمة الإماراتية أبوظبي: "من المعروف أن سكان الشرق الأوسط ممثلون تمثيلاً ناقصًا في الأبحاث المتعلقة بالأمراض، وأنه لا توجد أبحاث حتى الآن تشير إلى تأثير الطب الدقيق في دولة الإمارات. ولكنني أعتقد أن هذا الأمر ما زال سابقًا لأوانه؛ لأننا لا نمتلك بيانات كافية عن الجينات الوراثية للشريحة السكانية العامة في هذا الجزء من العالم، وبالتالي فإن هذه هي الخطوة الأولى بالفعل". 

وأضافت الدكتورة سارة: "على سبيل المثال، إذا تبيّن أن طفرة جينية مختلفة تُسبب سرطانًا معينًا في المنطقة، مقارنةً بالسكان المنحدرين من أصل أوروبي، فقد يمثل هذا هدفًا جديدًا لعلاجات السرطان الجديدة للسكان المحليين. أو إذا ظهر عقار جديد في السوق، فما هو تأثيره على السكان المحليين؟ ومن ثم، فإن تطوير أبحاث الجينات الوراثية والأمراض في دولة الإمارات والوطن العربي أمر مهم للغاية. ومن أجل حدوث ذلك، يجب أن يكون هناك تركيز على تمويل الأبحاث، إلى جانب وضع سياسات متعلقة بابتكار الأبحاث والرعاية الصحية لحماية المرضى والعلماء والأطباء على حدٍ سواء".

ومن المنتظر أن يجتمع خبراء التشخيصات الجزيئية والجينات الوراثية من منطقة الشرق الأوسط وشمال إفريقيا في الدورة المرتقبة من مؤتمر ميدلاب الذي ستنعقد جلساته في الفترة من 4 إلى 7 فبراير 2019 بمركز دبي التجاري العالمي لمناقشة موضوعات من قبيل العوائق التي تحول دون توافر الطب الدقيق في الوطن العربي، وإستراتيجيات الوقاية من الأمراض الوراثية، وتسلسل الجيل القادم في علم الأورام، وغير ذلك.

But even beyond this, proper flushing of the cooling circuit is the basis forlonger engine service life. This should take place at the latest when the water pump is replaced in order to remove particles that can be caused by corrosion or limestone. The Hamburg-based manufacturer MEYLE now supports workshops with a special tool. With this tool, which MEYLE offers in a joint cooperation with its partner HAZET, the cooling system can be cleaned with the aid of water and compressed air – completely without aggressive chemicals and cleaners. MEYLE shows how the engine cooling circuit can be cleaned in five simple steps.



Step 1: Preparation

The engine must be completely cooled down before starting work. The old coolant is then drained and the coolant expansion tank cleaned or replaced if heavily soiled. For environmental reasons, it is important to dispose of the waste liquid properly.



Step 2: Flushing the cooler

Remove the upper and lower radiator hoses on the radiator. The lower radiator hose is thoroughly rinsed from bottom to top for about two minutes with the radiator cap closed using the MEYLE tool. Short pulses of pulsating air into the system increase the effectiveness. The upper radiator hose can then be flushed from top to bottom until the outflowing water is clear and deposits have been removed.









Step 3: Flush engine block

Next, remove the radiator hoses. Using the MEYLE cooling circuit flushing tool, the upper radiator hose is thoroughly flushed until the outflowing water remains clear and any deposits have been removed. Also in this step short pulsating air blows increase the effectiveness of the rinsing process.



MEYLE workshop advice: If the thermostat blocks proper rinsing, it must be removed for rinsing. In this context, MEYLE recommends a general replacement of the thermostat.



Step 4: Flush the heat exchanger

If possible, the circuit to the heat exchanger should also be flushed until the outflowing water is clear. As a general rule, flushing the radiator, engine block and heat exchanger removes deposits of corrosion, limestone and silicone seal residues from the cooling system. These can accumulate over many years and permanently impair the functionality of the system.



Step 5: Refill the system

In the final step, all hoses, the drain valve and the thermostat are reattached. It must be checked whether all necessary connections have been made again. The cooling system can then be refilled with the coolant specified by the manufacturer. The mixing ratio should, for example, be checked with a refractometer. Finally, the system should be properly vented. A final check ensures that there are no leaks.

This ensures that all components of the flushing circuit, such as the water pump, function continuously and reliably.
