Top 10 Patient Safety Tips for Medical Centers





Dubai, United Arab Emirates, 06 November 2019: Dr Samir Shureih, MD says that patient safety is the mission of the American Association for Accreditation of Ambulatory Surgery Facilities (AAAASF) and the priority of your center. AAAASF has compiled a list of tips that can help ensure the highest level of patient safety within your facility.


Utilize a written screening protocol for venous thromboembolism risk for each surgical patient. The completed assessment should be placed in the medical record for each patient. A copy of the protocol and assessment tool should also be placed in the facility manual for reference.


Ensure each operating surgeon or dentist reviews the anesthesia plan and acknowledges the agreement in the medical record of any patient scheduled for a procedure.


Confirm medical records contain patient acknowledgment that the patient Bill of Rights was reviewed and understood by the patient or legal representative.


Ensure sequential compressive devices are used for surgeries of 1 hour or longer, except for procedures under local anesthesia.


Verify that a standard defibrillator or an automated external defibrillator unit is present and checked at least weekly as well as every surgical day for operability, with test results kept for a minimum of 3 years.


Be sure the instrument preparation and assembly area (clean utility area) is separated by walls or an adequate amount space from the instrument cleaning area (dirty utility area), or make sure there is a policy to clean and disinfect the dirty utility area before preparing and assembling packs for sterilization.


Check that there is always a minimum of twelve vials of Dantrolene stored in the facility if triggering agents are present.


Review your anesthetic care plan process to ensure it is based on the patient’s medical examination and assessment of any physical conditions that might affect decisions about preoperative risk management.


If you use a gas sterilizer, confirm that your personnel are badge tested for ethylene oxide exposure.


Verify the Informed Consent Form, which provides consent for administration of anesthesia or sedatives under the direction of the surgeon, dentist, or anesthesia provider, is completed for each patient scheduled for a procedure.

دبي، الإمارات العربية المتحدة، 06 نوفنبر 2019: يشرح الدكتور سمير الشريح أن سلامة المرضى مهمةً محورية للجمعية الأميركية لإجازة مرافق الجراحة الإسعافية" (AAAASF)، وكذلك أولوية للمركز الطبي الخاص بك.. وانطلاقاً من ذلك، قامت الجمعية بإعداد هذه اللائحة بأبرز النصائح التي يمكن أن تساهم في توفير أعلى مستوى لسلامة المرضى داخل منشأتك الصحية:


1- ملء استمارة البروتوكول المكتوب عن خطر الجلطات الدموية الوريدية عند أيّ عمل جراحيّ، وحفظ التقييم الكامل في السجل الطبي الخاص بكل مريض، ووضع نسخة من هذا البروتوكول وأدوات التقييم ضمن أرشيف المنشأة، بحيث يمكن العودة إليه عند الحاجة. 

2- التأكد من قيام كل طبيب جرّاح، أو طبيب أسنان، بمراجعة احتمالات تعرّض المريض لأيّ مخاطر للتخدير، وتدوين ذلك في السجل الطبي الخاص بالمريض المقرر إجراء عملية جراحية له.

3- التثبّت من تضمّن السجل الطبي للمريض إقراراً منه بأنه جرت مراجعة وثيقة حقوقه وفهمها جيداً، من قِبله أو من قِبل ممثله القانوني.

4- التأكد من استخدام أجهزة الضغط المتسلسل عند إجراء عمليات جراحية تستغرق ساعة أو أكثر، ما عدا تلك التي تتم تحت تخديرٍ موضعي.

5- التحقق من وجود جهاز تنشيط القلب أو الوحدة الآلية لوقف اضطرابات القلب، والتأكد بانتظام من أنها تعمل بصورةٍ سليمة، مرة كل أسبوع وكذلك في كل يوم يشهد إجراءً جراحياً، مع حفظ نتائج الكشف في ملف خاص لمدة 3 سنوات على الأقل.

6- التأكد من أنّ منطقة تجهيز وجمع الأدوات (منطقة المرافق النظيفة) معزولة بجدران، أو أنها بعيدة بما يكفي عن مساحة تنظيف الأدوات (منطقة المرافق القذرة). أو اعتماد تنظيف وتعقيم منطقة المرافق القذرة، قبل تجهيز وجمع الأدوات التي تحتاج إلى تعقيم.

7- التحقق المتواصل من وجود ما لا يقل عن 12 قارورة من مادة "دانترولين" الخاصة بترخية العضلات، ومن تخزينها بالشكل السليم في المنشأة، في حال وجود معدات لنقلها. 

8- قيام الطبيب المختص بمراجعة مسار عملية التخدير، للتأكد من أنها تستند إلى الفحص الطبي الذي أجري للمريض، وتقييم أي حالة جسدية قد تؤثر على القرارات ذات الصلة بإدارة المخاطر المعتمدة قبل الجراحة.

9- إذا كنت تستخدم جهاز التعقيم بالغاز، عليك التأكد من خضوع معاونيك لكشف التعرض لأكسيد الإيثيلين.

10- التحقق من الاستمارة التي تشمل موافقة المريض على مسار التخدير أو اعتماد المهدئات تحت إشراف الجرّاح أو طبيب الأسنان أو طبيب التخدير، قبل إجراء أيّ عملية جراحية.
